Job Overview

The clinical practice nurse will be a member of the collaborative team of the Partnership Comprehensive Care Practice - providing care for patients with HIV and/or Hepatitis C. The primary role will be to support patient education regarding medications, access to medications, and adherence to medications and appointments. The nurse will also assist in triaging patient visits and calls as well as assisting in patient paperwork and forms for collaborating agencies.

Qualifications:

Pennsylvania RN or LVN license. BSN from accredited school of nursing preferred. At least one year experience in a healthcare setting providing care to people infected with HIV is desired. Excellent oral and written communication skills. Basic patient assessment skills, specifically concerning HIV disease and its complications. Knowledge of electronic medical records, IDX registration and billing sys tem and Microsoft Office is a plus

Essential Functions

1 - Provide education and adherence counseling to patients. a. Coordinates with the Pharmacist to provide education on medications, to assess nd document patient's adherence to medication and follow up plan in the EMR. b. Assist the Pharmacist in accessing medications for patients through prior authorizations, local, state and federal programs. 2 - Provision of General Nursing support and services to HIV and/or HCV infected outpatients. a. Triage patient and respond to patient phone calls as needed. b. Call patient with lab results and routing imaging results. c. Assist with patient paperwork. 3 - Administrative and Clinical Activities. a. Participate in Quality Improvement Committee. b. Participate in other internal and/or external committees as directed. c. Document in CAREWare as needed for Ryan White Grant requirements. d. Initiates and performs ongoing review of policies related to services provid ed. 4 - Performs other duties as assigned.
