UPMC Presbyterian is hiring a Casual Unit Based Clinical Pharmacist to help support the PUH-Pharmacy Department for its location.

Under the general direction of the Pharmacy Operatio! ns Manager, the Unit Based Clinical Staff Pharmacist assigned to Unit Based Pharmacy Practice functions as a member of a multidisciplinary team to provide comprehensive patient care services, both clinical and operational. The Clinical Staff Pharmacist has responsibilities that go beyond, but may also include, providing routine pharmacy services (retrospective order review, verification of medication orders, and dispensing of medications for patients). These individuals:

  • Spend at least 50% of their time in their area of specialization (e.g. critical care, oncology, etc.)
  • Participate in multidisciplinary teams and proactively provide recommendations for patient therapy prior to orders being written by physicians.
  • Provide direct input to key decisions regarding medication treatment guidelines, protocols, drug formulary decisions and dosing guidelines.
  • Act as the drug therapy expert in their area; referred complex questions regardin! g patients therapy and reactions to medications from other mem! bers of the health care team (pharmacists, nurses, physicians, etc).
  • Participate in multidisciplinary quality improvement committees that focus on improvements to safety and overall outcomes for their patients.
  • Instruct and educate patients, as well as other health care workers regarding medication therapies to optimize patient outcomes.
  • Support drug related research.
  • These services will be provided in compliance with federal and state regulations; JCAHO standards; and health system, hospital and department policies. The Clinical Staff Pharmacist will be involved in patient care activities supporting a variety of patient populations, ranging from geriatrics to pediatrics. Inpatient and outpatient practice areas include, but are not limited to general medicine, surgery, intensive care, transplantation, orthopedics, oncology, and cardiology.
Responsibilities

  • The Unit Based Clinical Pharmacist must demonstrate be! haviors that support the Mission, Vision, and Values Statement of UPMC Presbyterian/Shadyside, the Department of Pharmacy and Therapeutics, the UPMC Health System, and contribute to the overall success of our organization.
  • Provide comprehensive pharmacy services to assigned Unit(s), including, but not limited to: medication order entry and verification, medication profile review for appropriateness, clinical cognitive services as needed by the specific patient population and procurement of the medication product in a safe and timely manner.
  • Participate in development and implementation of new Department of Pharmacy and Therapeutics initiatives by reviewing literature, preparing educational materials, determining implementation strategies, designing informatics, creating outcome measures and monitoring forms, and/or designing competency assessment tools.
  • Screen all medication orders for adverse drug reactions/medication occurrences as per Departmen! t standards and reports through appropriate reporting process.
  • Develop and implement initiatives that enhance patient safety in their area(s) of responsibility.

    • Document monitoring and evaluation through the pharmacy computer system, THERADOC and DUDSM Program monitoring forms.
    Provide educational in-services to pharmacy, nursing, and/or medical staff related to Department of Pharmacy and Therapeutics initiatives or clinical programs.

    Mentor pharmacy students and/or residents to facilitate ongoing teaching programs and support Department and School of Pharmacy missions.

    • Demonstrate continuous professional development by maintaining a portfolio of items of interest for educational and professional growth interests.
    • Direct pharmacy technicians to assure the timely, safe and appropriate delivery of medications to the unit(s).
    • Verify Labor and Delivery patient orders for UPMC Horizon
    Basic Qualifications

    • B.S. degree in Pharmacy or a Ph! armacy degree from an accredited School of Pharmacy. Good communication skills.
    • 2 years experience in hospital pharmacy or one-year accredited Residency.
    • Good written and verbal communication skills, must work well independently, and demonstrate leadership in their current position.
