A part time (28 hours per week) Care Manager / Dental Coordinator position is available to support the Pediatric Offices within the Charles Cole Medical Group.
The Care Manager/Dental Coordinator will f acilitate the processes on a daily basis to coordinate pediatric patient care across a continuum of care in a timely and cost-effective manner for special needs children; will coordinate input from all health care professionals necessary to best meet the patients’ physical, emotional and spiritual needs; will assist patients t! o make sure an optimum outcome is achieved through assessment ! of patient/family needs, coordination of services, monitoring and evaluation; and will create and maintain practices to find better ways to care for complex and chronically ill children and to effectively transition them to adult care.
The qualified candidate must be a current Registered Nurse, License Practical Nurse, or a Social Worker licensed in the state of Pennsylvania or in a health service related field. Excellent communication and organizational skills with the ability to promote teamwork within and across departmental boundaries is required. Chronic care expertise is required.
