Concordia Visiting Nurses is looking for Patient Care Coordinators to become part of our home care team. The Patient Care Coordinator (PCC) is responsible for monitoring and evaluating the care and service s provided to our patient s from admission to discharge. The PCC will promote quality cost-effective outcomes to obtain and ensure the best patient outcomes. Candidates must have critical thinking skills to resolve complex care issues, leadership qualities to lead a team of clinicians , and excellent communication skills to interact with pati ents, famil! ies and physicians. The PCC will participate in the administra! tive on-call rotation as well as provide in-home nursing visi ts on a monthly basis and as-needed .
Requirements:
RN with a current valid Pennsylvania license
Chronic Care Certification within one year of being employed in the PCC role
OASIS certified within one year of being employed in the PCC role
Proficient and knowledgeable of electronic medical record s
Must have current and valid driver’s license and automobile insurance coverage
