NURSE CASE MANAGER (OUTCOMES MANAGEMENT)
FULL AND PART TIME OPPORTUNITIES AVAILABLE â€" DAY/EVENING SHIFT

Job Summary: The Nurse Case Manager is responsible for the comprehensive, interdisciplinary coordination of patient care for targeted patient populations in order to ensure that quality care and patient outcomes are achieved within established time frames. This individual performs comprehensive patient assessments and interviews, resource management, utilization management, discharge planning, and facilitates referrals to other levels of care. This individual works collaboratively with the multidisciplinary care team to facilitate achievement of desired treatment outcomes, to assure access to appropriate levels of service within Pinnacle Health and efficient use of economic, technologic, and human resources throughout a patient's hospitalization.
- Implement coordinated, interdisciplinary plans of care
- Serve as a clinical resource in identified specialty areas
- Work closely with the Medical Staff to develop clinical order sets
- Serve as patient advocate
- Analyze data and develop strategies to improve care delivery and function as an integral part of the care delivery team
- Current Pennsylvania Registered Nurse license
- 3-5 years of clinical nursing experience required (Med/Surg, Critical Care, Emergency, Acute Care)
- Basic Life Support (BLS) certification or successful completion of BLS training and certification upon hire
- Literate in use of computer software and information systems in order to manage patient records
- Bachelor Degree in Nursing
- Case Management experience preferred
- Case Management Certification
