The RN Nurse Navigator acts as a health advocate to help consumers receive the health care and behavioral health care they need in addition to assisting in coordinating hospital care and discharge to the commu nity, arranging travel to appointments, assists in development of personal plans for consumer health collaboration and wellness.

- This position requires a licensed Registered Nurse
- A minimum of one (1) year current med/surg nursing experience
- A minimum of three (3) years mental health experience
- In addition, a current and valid Pennsylvania driver's license and reliable transportation may be required
