Chartis has an opportunity for a Case Manager at Health Direct, Inc.(HDi), a Chartis company, that provides medical management services to its customers.

The key responsibilities for this position are as follows:

• Provides medication management of clinical escalation alerts including investigation of medication details, injured workers' medication history, communication with prescribing providers and comprehensive analysis in preparation for medical director review. Particular emphasis on management of opioid medications with follow-up to ensure appropriate medication tapering and/or weaning is occurring. High level of attention to detail required with extensive medical record analysis. Analysis of other medical care aspects of long-term, complex and/or catastrophic claims included

Position Requirements

• Licensed Professional Registered Nurse required, CCM and CNLCP (Certified Nurse Life Care Planner) preferred.
• Minimum 5 years experience in a managed care or healthcare setting, preferably within a workers’ compensation or disability management setting.
• Excellent communication, analytical and organizational skills required.
• Proficiency in the use of computers required.
• Requires independent judgment, discretion, poise, diplomacy, courtesy and the ability to perform at peak levels in a dynamic, growing environment
