Tracking Code 25825-GB Job Description Home Health LPN - PRN VNA - Gettsyburg, PA About the position Provides comprehensive family oriented nursing servic! es to individuals in the home setting by use of the nursing process in accordance with physician orders, the established plan of care, corporate policy and procedures, and the Nurse Practice Act. PERFORMANCE EXPECTATIONS: A Related to Patient Care 1 Participates in the development of a written plan of care in accordance with the patients diagnosis, patient and family needs, physicians orders, and the corporate policies and procedures. 2 Provides selected nursing procedures and treatments to patients as outlined in the plan of care and assigned by the primary nurse. 3 Notifies physician and other care agencies of changes in patients condition and care needs and obtains new orders, as needed.
4 Assists with coordination of patient care through dialogue with other disciplines involved with the patient and documents same. 5 Participates in patient care conferences. 6 Documents observations and intervention in the patients clinical record in a timely, legible, and accur! ate manner. B Related to Staff 1 Functions as a member of an i! nterdisciplinary team, encouraging and maintaining open communications at all times.
2 Fosters a climate of mutual respect and teamwork among co-workers and management. 3 Assist in the orientation of new staff. C Related to Corporate Management 1 Submits patient visit schedule to Scheduler and communicates significant changes to Primary Nurse. 2 Functions in a cost-effective manner in regard to utilization of time, mileage, supplies, etc.
3 Meets or exceeds established productivity and quality assurance standards. 4 Seeks consultation, guidance and direction from professional or administrative staff, as needed. 5 Informs manager of perceived problem areas related to patient care, corporate management, or community relations. 6 Keeps clinical skills current with nursing practice norms.
7 Exhibits a comprehensive knowledge of, commitment to and demonstrated ability in the practice of home health nursing. 8 Exercises sound judgment and works effectiv! ely under pressure. 9 Demonstrates a working knowledge of skills and services provided by each member of the multi-disciplinary team and the ability to relate these to identified patient needs. 10 Acquires and utilizes knowledge of community resources.
11 Elicits feedback from appropriate sources regarding the quality of services provided. D Related to Community 1 Demonstrates a professional attitude, sound judgment and continuity of interactions with physicians, other health care providers and community agencies. 2 Represents the VNA at community activities, as delegated. 3 Promotes a positive corporate image.
4 Acquires and utilizes knowledge of community resources. E Common Expectations 1 Maintains established policies and procedure, objectives, quality assessment, safety, environmental and infection control standards. 2 Maintains confidentiality of patient and organizational information. 3 Provides outstanding service to all customers; fosters teamwork! ; and practices fiscal responsibility through improvement and innovatio! ns.
4 Demonstrates a commitment to patient, visitor and staff by: complying with all applicable safety regulations; learning the impact of medical errors and methodology that will lead to reduction of errors; reporting actual and potential errors, as well as hazardous conditions; identifying opportunities to standardize processes and error proof systems that will lead to increased safety; and participating in safety education programs and root cause analysis as required. 5 Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops. 6 Attends meetings as required. 7 Performs other related duties as assigned.
QUALIFICATIONS: Graduate of a practical nurse program. Licensed as a practical nurse in the Commonwealth of Pennsylvania. Minimum of one (1) year in an acute care setting is preferred. Previous home health experience desirable.
Current CPR Compu! ter skills preferred Valid drivers license Able/willing to drive in varying weather conditions and in urban and rural areas.
