UPMC/Jefferson Regional Home Health is seeking a full-time RN to provide skilled nursing services to patients in their homes in Brentwood, Brookline,! Beechview, Dormont and surrounding communities. The general work hours are Monday through Friday 8:00 a.m. to 4:30 p.m. with rotating weekend requirements. Responsibilities

• Provide direct patient care, evaluate outcomes, and adjust nursing care process as indicated
• Establish plan of care in collaboration with physician, patient and UM professional
• Complete visit assessment, OASIS, 485/ verbal order documentation via laptop PC

Basic Qualifications

• Graduate of an accredited school of professional nursing is required
• Minimum one year of RN experience is required
• A BSN is preferred
• Medical-surgical or home care RN experience is preferred

Licensure/Certifications

• Graduate of an accredi! ted school of professional nursing is required
• Minimu! m one year of RN experience is required
• A BSN is preferred
• Medical-surgical or home care RN experience is preferred
