Is recruiting for two Internal Medicine or Family Practice physicians to join our Primary Care team. We are seeking candidates with excellent cl! inical skills and experience in the full range of primary care; including gender specific care for female Veterans, managing acute and chronic medical conditions such as diabetes, HTN, hyperlipidemia, and pain. Our team is set up with the principals of Patient Centered Medical Home Model of care practicing team approach to handle the care needs of our patients to improve health outcomes and quality of life. The Primary Care Physician will be joining an interdisciplinary team consisting of other primary care providers, nurses, social workers, clinical pharmacists, nutritionists, psychologists, and psychiatrists who constitute our dynamic Primary Care Clinic. Each provider will be assigned an RN, other nursing and clerical staff to help support team functions to serve a panel of about 1,200 patients. There will be Saturday clinic coverage responsibilities as part of a 40-hour a week work schedule, with no on-call duties. This is an outpatient only responsibility.
