Job Summary: - To coordinate services, programs, and personnel necessary to facilitate the patient through the continuum o! f care. - Facilitates and insures the achievement of goals set by the Case Management/Performance Improvement programs such as quality patient care, timely and appropriate resource use and minimal financial risk. - Collaborates closely with the health Care Team to provide a coordinated experience to patients and families.

Skills & Abilities: Care Coordination Works Independently with minimal supervision.. Exhibits sound decision making and organizational skills and has the ability to set priorities.. Possesses excellent interpersonal skills . Exhibits ability to negotiate and work collaboratively to multifaceted health care team. Assumes direct responsibility in the management of the facility length of stay, cost per case and Utilization compliance on an ongoing basis.

Qualifications

Training & Education: Basic educational qualification for professional affiliation required. Bachelors degree in a Nursing or equivalent training and pr! oven experience with applied practice in an appropriate health! care discipline. Education Preferred: Bachelors degree in Nursing and specialty certification preferred.

Professional Requirement: Current RN licensure in the state of Pennsylvania

Experience: Strong, 2 years, acute care background required. Case Management /Utilization review experience preferred Ability to prepare reports and formal presentations preferred.
