The Transitional Nurse Liaison is responsible to coordinate all offered services for home healthcare as applicable. This may include completing part or all! of the admission process for patients. Partners with the sales team in developing and maintaining referral source relationships. Their main responsibility is discharge planning to the agency from facilities and marketing home care services to referral sources .
QUALIFICATIONS

  • Current Pennsylvania Licensed Registered Nurse . · A minimum of three (3) years nursing experience required. Supervisory and acute care discharge planning experience preferred.
  • A minimum of two years ( 2 ) years medical marketing experience r equired
  • Maintains current PA Driver’s License and auto insurance.
  • Strong clinical assessment skills.
  • Skills in typing and organizational abilities.
  • Preferably has home healthcare experience.
  • Able to work independently, demonstrating good judgment.
  • Must be accurate and detail oriented.
  • Practices good customer service skills in all working relationships.
  • Strong inte! rpersonal skills, respectful, and courteous nature.
  • A! bsence of criminal background.
DUTIES AND RESPONSIBILITIES

A. Provides on-site consultative services to area hospitals and facilities regarding adult and geriatric patients referred for homecare services, and referrals for post acute rehab and skilled nursing facility ( SNF ) care.

B. Provides comprehensive adult and geriatric assessments to assure development of thorough homecare plans or discharge plans to Concordia Visiting Nurses. Utilizes strong clinical assessments skills to facilitate the development of plans for patients of all ages and medical needs.

C. Coordinates/documents adult and geriatric assessments prior to discharge to Concordia Visiting Nurses.

D. Coordinates homecare or post-acute skilled nursing needs to assure quality patient care and delivery of services: helps promote information transfer between levels of care.

E. Attends interdisciplinary clinical meetings at institution as requested.

! F. Demonstrates timely and accurate computer documentation/written documentation of patient information as well as daily activities to Concordia Visiting Nurses branch staff via the contact relationship management software ( CRM ) .

G. Provides timely response to all intake inquiries at assigned institution; works closely with Intake and Clinical Management.

H. Identifies, develops and builds upon referral sources within assigned institution; develops relationships with other referral sources in market area.

I. Assists in the identification of new services that are needed or should be expanded based on experience/input at institution.

J. Educates markets and promotes to patients, families and referral sources the Concordia Lutheran Ministries continuum of care.

K. Provides education, supervision and counseling to the adult and geriatric patient/family regarding expectations of homecare plans and discha rge to SNF for post-acute ! care.

L. Performs work under the supervision of the Agency Man! agement .

M. Answers the phone, takes inquiries or messages using professional telephone technique.

N. Attends required and appropriate office meetings.

O. Maintains a good working relationship with all staff.

P. Prepares and places ads for recruitment of clients.

Communication
A. Communicates potential risk management issues and reports abuse or unsafe â€" illegal practices as required by law.

B. Willing to participate in the marketing efforts of the office and does so as assigned.

C. Participates in community relations’ activities as directed by the Agency Management.

D. Promotes the Agency through positive representation and communication of its services.

E. Communicates effectively with all team members.

F. Retains positive, open communication with the Agency Management.

G. Works as a positive, effective team member.

H. Recommends new policies and/or changes! in current policies to improve quality of patient services as well as efficiency and effectiveness of services.

I. Follows all infection control and OSHA standards and all state and federal regulations.

J. Is punctual and reports to work as scheduled.

K. Is flexib le and works cooperatively for A gency needs.

L. Understands the concept of continuous quality improvement.

M. Applies the concepts of quality improvement to all work practices.

N. Knowledgeable of the Agency’s Quality I nitiative goals and participates as directed.
