Location: Coaldale Pennsylvania

Description: St. Luke's Hospital & Health Network is currently seeking to employ LPN D right now, this position will be placed in Pennsylvania. WORK PERFORMED

JOB DUTIES AND RESPONSIBILITIES:
1. Follows established performance standards and implements facility policy and procedures. 2. Identifies and reports safety hazards and initiates corrective action. Follows policy at all times. 3. Follows, directs activities toward, and implements Residents’ Rights. 4. Participates in facility and outside educational programs and attends staff meetings. 5. Participates in interdisciplinary team and rehab meetings. 6. Administers treatments and medications according to physician’s orders and accurately and in a timely manner records the treatments and medications on appropriate forms. 7. Prov! ides clinical supervision to certified nursing aides in activi! ties of daily living and assists the CNA with proper preventive safety measures and in the use of adaptive equipment. 8. Assumes and reports to the RN changes in resident/patient condition. 9. Documents resident/patient care provided and resident’s/patient’s response or lack of response to interventions and the treatment provided. 10. Assesses and reports to the RN changes in resident/patient condition. 11. Assists with admission/transfer/discharge of residents/patients. Completes admission/transfer/discharge documentation accurately and thoroughly. 12. Performs LPN nursing skills according to current standards of practice. Takes vital signs, heights, weights, does enemas, maintains and inserts IV catheters. 13. Administers special care as: tracheostomy and colostomy care, and suctioning. 14. Provides direct resident/patient care as needed. 15. Continually assesses resident/patient needs in conjunction with the RN, and initiates and contributes to the development of the! resident/patient care plans as necessary. 16. Works cooperatively and courteously with residents/patients, administration, nursing personnel, physicians, families, consultant personnel and ancillary personnel. 17. Renders immediate assistance to any resident/patient in need or carries out all ADL activities. Answers resident/patient call lights in a timely, courteous manner. 18. Actively listens to resident’s/patient’s family or physician concerns and report special problems to the RN or DON. 19. Comes to work as scheduled and consistently demonstrates dependability and punctuality. 20. Accepts assigned duties in a cooperative, respectful and courteous manner and performs other related duties as directed by the RN or DON. 21. Observes infection control and safety policies and procedures. 22. Assists residents/patients with ambulation, ROM, dining, toileting and transferring needs. Assists Restorative as directed. 23. Observes and documents correct food and fluid intake! . Reports and documents any noted dining problems. 24. Starts IV’s af! ter completion of an IV course with certificate of course completion and maintains yearly knowledge of and skill competencies according to Geriatric Center policy. 25. Completes all assigned monitoring and evaluation Performance Improvement activities accurately, timely and in a thorough manner. 26. Has a customer minded service perspective. Displays respect at all times. 27. Is cheerful and maintains a positive work attitude. 28. Maintains responsibility for own personal development through active participation in inservices and seminars. 29. Complies with established chain of command for work related problems. 30. Functions as a role model, resource person and preceptor for orientation of new personnel. 31. Implements and documents resident/patient care interventions and the outcomes of these interventions in accordance with the plan of care and established policy and procedures. 32. Takes an active role in performance improvement activities. 33. Role models the Geriatric! Center’s core values and customer service behaviors in all interactions.

Job Qualifications:
EDUCATION:
Current Pennsylvania License. High school graduate.

Graduate of accredited Pennsylvania nursing school for LPN’. Completion of an accredited pharmacology course with Certificate of completion NAPNES Certificate preferred. Knowledge of growth and development of patients of all agesâ€"from pediatric to geriatric. Possesses and maintains a current CPR Certification.

TRAINING AND EXPERIENCE:
Prior experience in geriatric care or medical-surgical experience with a geriatric populace is desirable.

SPECIAL CONSIDERATION FOR LICENSED PRACTICAL NURSES:
1.

Must be under the supervision of an experienced RN at all times.2.

Needs to successfully complete the State Board Examination within sixty days after graduation.

WORK SCHEDULE:
Day, middle or night shift with! varying hours based on the needs of the department.
