Looking for seasoned RNs to provide telephonic patient support to achieve and/or maintain patient’s optimum health status through best-practice disease surveillance ! and early intervention of care coordination, education, and support. Applicant should be interested in working to improve the engagement and outcomes of patient with chronic disease through self-management support and behavior change techniques.
Candidate must be a self-motivated, conscientious professional with excellent oral and written communication skills; the ability to establish and maintain positive relationships with patients over the telephone; and the ability to represent the organization to external clients and vendors with confidence.
Candidate must be a Registered Nurse with current Pennsylvania license in good standing with at least 2-3 years of acute care clinical experience; knowledge of population health and the management of chronic disease; and experienced in patient education and self-management techniques. Physician office practice and/or case management experience preferred. Competitive salary and benefit package.
