We assist older adults in maintaining quality, dignity and autonomy in their lives by supporting the family unit within! the context of a holistic and faith-based approach to medical and social services.

LIFE Pittsburgh provides guidance for older adults who wish to remain in their own home, but who have health problems that prohibit their ability to safely do so. Traditional and non-traditional services are combined to support the individual and encourage an independent lifestyle. Services are provided on an individual basis to tailor a flexible, yet comprehensive, medical and supportive plan of care.

Through the LIFE Pittsburgh program, frail and fragile seniors can remain in their own homes and receive a coordinated plan of care that enables them to maintain their independence and enjoy a safe and active life in the community.

We are currently searching for Registered Nurses to be responsible for nursing assignments of health needs and contributing to the plan of care for individuals and families enrolled in the LIFE Pittsburgh program. The Registered Nurse will! promote and maintain the health of LIFE Pittsburgh participan! ts in the community through teaching, counseling and appropriate preventive and rehabilitative services. Will be responsible for the initial implementation of nursing care plans and family nursing needs. Valid Pennsylvania RN License, PA Drivers License with access to a vehicle and geriatric experience in a home health or community setting.
