Directly responsible for the provision of nursing care an! d services that maintain comfort, safety, physical and emotional well-being and personal dignity. Duties are performed under the supervision of professional nursing staff, per established facility guidelines and procedures and as mandated by regulations. “Demonstrates competency in assessment and range of treatment/care appropriate to the age of the resident cared for:

RESPONSIBILITIES:

  • Refers to individualized care plan in completion of the resident care needs.
  • Performs services related to personal hygiene and care: bathing, oral care, hair care, skin care, toileting, etc.
  • Performs services related to feeding and reports information including intakes, outputs, and appetites as required.
  • Monitors and reports changes in resident condition and other information to Team Leader/Nurse Manager/Supervisor.
  • Takes and records vital signs: Temperatures, pulse, blood pressures, respiration.
  • Performs resident t! ransfers per facility lift and care plan procedures.
  • ! Assists residents with positioning, mobility, ambulation and transfer utilizing appropriate equipment, aids and/or lifting procedures. Familiar with individual needs of assigned residents regarding transfer/mobility/transport criteria.
  • Performs Post Mortem Care.
  • Performs daily charting and update of care plan as needed including updates regarding residents’abilities and routines.
  • Communicates effectively with residents, staff and visitors.
  • Follows oral and written instructions, appropriately interprets resident care plans and maintains required documentation/records.
  • Performs bed making and basic cleaning/housekeeping functions related to resident care in assigned area, organizing residents’ personal belongings, cleaning of furniture, i.e. wheelchairs, recliners, equipment and area.
  • Performs laundering of residents personal items (hosiery).
  • Fills water pitchers.
  • Performs letter writing, reading, vis! it outdoors, friendly visitation, etc
  • Reports hazards and equipment malfunction to Nurse Manager/Supervisor in timely fashion.
  • Responds to call bells, chair checks, bed checks, and Roam Alerts promptly.
  • Attends daily report at change of shift as assigned.
  • Assists nurse with emergencies.
  • Assists with Therapeutic Programs including restorative nursing programs, i.e. R.O.M.s, ambulation, bowel/bladder training, etc.
  • Demonstrates competency in range of treatment/care appropriate to the age of the resident cared for: Aging population.
  • Performs other support services and duties as requested by the Team Leader, Nurse Manager, Supervisor, Assistant Director of Nursing, Administrator of Health Care Center and/or respective Vice President.
JOB QUALIFICATIONS: Must be a minimum of 18 years of age with the ability to read, write and speak the English language. Perform simple math calculations. Completion o! f State approved training program and successful testing. Current Penns! ylvania Registration required. Facility will test qualified persons who have completed required approved training program. Orientation and on-the-job training is provided. Interpersonal abilities required and ability to control reactions toward undesirable behavior patterns of residents in facility’s care. Experience or familiarity with a geriatric setting and related equipment and processes preferred.

WORKING CONDITIONS: Frequent standing, walking and twisting; occasional sitting, squatting and kneeling and infrequent crawling. Capable of lifting minimum of 50 pounds and/or capable of transferring persons with weight of 120 points or less; and/or capable, with the assistance of anotherperson(s), transferring residents who weigh more than 120 pounds. Indoor, well-lighted and climate controlled environment. Occasional outings may expose to outdoor climate elements. Minimal exposure to noise. May be routinely required to utilize personal protective equipment! while using chemical cleaning agents or practicing standard precautions. Frequent exposure to odors. Seldom exposed to fumes. Needs to develop awareness and appropriate responses to occasional exposure to physical assault when dealing with hostile or combative residents. MSDS (Material Safety Data Sheets) are available in case of an accident with chemicals.
