Develop strong relationships including Senior Leadership, the Discharge Planning and Case Management tea! ms, Nursing, Physicians, and key post-acute providers

Artfully integrate the Care Transitions Model into the current work flow and care management activities

Identify the high-priority care management changes that will be required to improve performance in the company’s key leverage areas

Manage a team of Transitional Care Specialists who are focused on the bundled payment program

Establish and achieve quality improvement and cost reduction targets

Become a key member of the Remedy Partners Care Management Team, helping to design and continuously learn and improve the approach to care management, bundled payments, analytics, relationship development, and overall healthcare improvement

Required Skills

RN

MSN, MPH or MBA

Progression from Clinical Nurse (med/ surg or ED) to running Operations and managing staff and interfacing with leadership

Excellent leadership skills
