Description: Life Choice Hospice is presently looking of Licensed Practical Nurse - Bucks/Montco right now, this position will be placed in Pennsylvania. The License Practical Nurse (LPN) supports the RN Case Managers in the coordination of the individual patient care activities of the interdiscipli! nary team. The LPN provides holistic nursing care to terminally ill patients and their families, which includes appropriate support for families at time of death. Nursing care is based on the nursing process, as directed by the RN Case Manager, which consists of implementation of interventions, providing patient education and evaluation of patient outcomes all within the scope of the LPN Practice Act.
Job Qualifications:
Minimum of two years experience as a Licensed Practical Nurse required; background in long term care or oncology nursing preferred. Graduate of an accredited LPN school of nursing required. Licensed as a Licensed Practical Nurse in the state of practice.
